The easiest way to obtain results is to go directly to your Patient Account which will provide detailed instructions for viewing your results online.
For more information regarding medical records, or to obtain records, visit the Health Information Management Services (HIMS) department at any Banner Health hospital. Banner hospital locations can be found HERE. Please have your government-issued photo ID available. If you are requesting records on behalf of a patient (i.e., personal representative, patient advocate, executor, trustee, or guardian), you will be asked to provide additional documentation to verify that you are legally authorized to make this request.
